I found several of these cards; all without the Y Cable.
I got tired of trying to find one so I decided to source the proper cabling and recreate the exact cable myself using Pinouts from the Apple Service manuals.And that is what I have listed here! A brand new 2023 Recreation of the Apple 590-0703-A Cable.
These cables are made by me with high end cabling and have been tested to work on actual Apple IIe PDS cards.
A bunch of games tested on Joystick port and ADTPro used to format, and write to floppy drives. 100% working! This cable has a DB19 Pin female connector for an External Apple II floppy drive (or smartport device), and a DB 9 Female connector for a joystick.
